Q: My doctor need my records urgently, how can he or she get them?

Call 214-347-4012 or ask your physician to send a request on letterhead to fax number 512-551-0647.

Q: Can medical records be faxed to me?

No, due to the confidentiality of the records we are not able to fax records to patients.

 Q: Can I access my medical records over the Internet?

Yes, go online to https://medrelease.healthmark-group.com/360. The website will provide instructions on how to fill out an authorization and have records available to you electronically.

 Q: Can my Physician access my medical records via the Internet?

Yes. Physicians on staff at Texas Health Southlake can gain access to your medical records using our CareGate Physician Portal.
